Volunteer Registration/Login

To best meet the needs of our students and teachers, a 3 tiered volunteer level system has been implemented. Please review the difference between the levels and select the option that best fits the volunteer opportunities you are seeking to participate in.

Level 1 Volunteers

A Level 1 volunteer position involves direct student contact under limited supervision. Examples include: PYP Exhibition volunteers, Head-to-Head volunteers, Reading help, field trip chaperones, club supervisors, and volunteers within the library, classroom, lunchroom, playground, or at special events.

Level 2 Volunteers

Level 2 volunteers have the same clearance as a Level 1 volunteer, but are qualified to drive students to activities off school grounds such as athletic and club events.

Level 3 Vounteers

Level 3 volunteer positions involve unsupervised, direct contact with students in various settings such as overnight school trips or volunteer athletic coaches.

Level 2 Volunteer Liability and Certification Statements (Drivers)

I understand, as a volunteer driver, I must be at least 21 years of age, be a non-student, hold a valid driver's license, and have the required insurance coverage in effect on any vehicle used to transport students. I further understand that my personal insurance is responsible for providing coverage for any claims relating to personal injury or property damage arising from ownership, operation, or maintenance of the vehicle used while driving on a field trip or extracurricular activity. I will upload images of my current vehicle registration, driver's license, and proof of insurance to my United for Success volunteer profile account.

Level 2 drivers must upload images of their current driver's license, registration and proof of insurance.

After your application is processed, you will be provided instructions via email as how to upload images of your driver's license, vehicle registration, and proof of insurance. You will also be responsible for keeping this documents up-to-date in your volunteer profile, prior to driving students.

Level 3 Volunteers are required to complete a Livescan Fingerprint Background Check Request form and a Waiver Agreement form. Volunteers are responsible for the cost. The required forms and further information will be emailed to the address provided on this account. You will need to print the forms, complete the process and return them to the Human Resources Department in the Midland Public Schools Administration Center on Carpenter Street.

Level 3 volunteers, planning to drive, are required to upload a current driver's license, vehicle registration, and proof of insurance. After your application is processed you will be provided instructions via email as to how to upload this documents.

By submitting this form, you give us permission to perform a background check on you. You will be notified annually via email about keeping your volunteer status. You will need to respond to the emailed survey to remain active in the program and current with a background check. Furthermore, you affirm that you have read and consent to the details of the Criminal History File Search.

A priority for Midland Public Schools is to ensure the safety and welfare of our students. Volunteer screening is a part of this effort. Individuals applying for Level 1-3 volunteer positions will need to complete this form in order to authorize a criminal history background check. Please complete this form and submit by clicking "Create Account". The MPS Volunteer Manager will process your application. Midland Public Schools will handle this information confidentially, like it handles employee personnel records. Once the background check is completed, you will be notified via email that you have been approved to participate as a volunteer. All MPS schools have access to the approved list of volunteers and can verify your status as well. If a question arises regarding the information received during the background check, you will be contacted directly. MPS must complete a background check on volunteers each year that you volunteer in our schools.

Please include any/all maiden and prior LEGAL names as this is required to complete a background check.

Background Information: An MPS volunteer application may be denied, if the applicant has been convicted of a felony or misdemeanor specifically listed in the Child Care Exclusionary Offenses Guide provided by LARA: The Michigan Department of Licensing and Regulartory Affairs.
