To best meet the needs of our students and teachers, a 3 tiered volunteer level system has been implemented. Please review the difference between the levels and select the option that best fits the volunteer opportunities you are seeking to participate in.

Level 1 Volunteers

A Level 1 volunteer position involves direct student contact under limited supervision. Examples include: after school assistance, club supervisors, day field trip chaperone, Hallway Hero, PYP Exhibition Mentor, Reading Buddy, Picture Parent, and volunteers within the library, classroom, lunchroom, playground, or at special events.

Level 2 Volunteers

Level 2 volunteers have all the clearance of a Level 1 volunteer, but are qualified to drive students to activities off school grounds such as day field trips or athletic events.

Level 3 Vounteers

Level 3 volunteer positions involve unsupervised, direct contact with students in various settings such as overnight school trips or volunteer athletic coaches.

Level II Volunteer Liability and Certification Statements (Drivers)

I understand that, as a volunteer driver, I must be at least 21 years of age, be a non-student, hold a valid driver's license, and have the required insurance coverage in effect on any vehicle used to transport students. I further understand that my personal insurance is responsible for providing coverage for any claims relating to personal injury or property damage arising from ownership, operation, or maintenance of the vehicle used while driving on a field trip or extracurricular activity. I will submit copies of my vehicle registration, driver's license, and proof of insurance to Midland Public Schools.

Level 2 drivers must submit their registration and proof of insurance.

After submitting this form, you will be provided instructions on how to upload a copy of your drivers license, vehicle registration, and proof of insurance.

Thank you for your interest in being a Level 3 Volunteer for Midland Public Schools. Level 3 Volunteers are required to complete a fingerprint criminal history background check called "Livescan." The first step is to come in to the MPS Administration Building (600 E. Carpenter St., Midland MI 48640) to get the current forms to be fingerprinted.

Next, take the Livescan fingerprint request form and valid picture identification to the Midland Law Enforcement Center, 2727 Rodd Street, Midland, Michigan. The Law Enforcement Center only completes this service on Thursdays between 9:00 a.m.-11:00a.m. at a cost of $50.75, which is only payable by cash or check, no credit cards. Volunteers are responsible for the cost of the Livescan background.

Please take the following items with you to the Law Enforcement Center:

  • Livescan Fingerprint Request Form
  • Cash or Check Payable to: The City of Midland ($50.75)
  • Photo Identification (license, passport)

Important: The completed Livescan Fingerprint Request form should be returned to Midland Public Schools Administration Building

All Level 3 volunteers are required to submit a form of photo identification (either a driver's license, state issued identification card, or passport), vehicle registration, and proof of insurance. After submitting this form you will be provided instructions on how to upload these documents.

By submitting this form, you give us permission to perform a background check on you each year unless you notify us not to do so. You will notified annually when your background check will be completed again. Furthermore, you affirm that you have read and consent to the details the Criminal History File Search.

Criminal History File Search Consent Information (click to expand)

A priority for Midland Public Schools is to ensure the safety and welfare of our students. Volunteer screening is a part of this effort. Individuals applying for Level 1-3 volunteer positions will need to complete this form in order to authorize to authorize a criminal history background check. Please complete this form and submit by clicking "Create Account". The MPS Volunteer Manager will process your application. Midland Public Schools will handle this information confidentially, like it handles employee personnel records. Once the background check is completed, your school will be notified that you have been cleared to participate as a volunteer. If a question arises regarding the information received during the background check, you will be contacted directly. MPS must complete a background check on volunteers each year that you volunteer in our schools. By signing this form, you give us permission to perform a back ground check on you each year unless you notify us not to do so.

Please include any/all maiden and prior married names as this is required to complete a background check.

Background Information: An MPS volunteer application will be automatically denied if the applicant has been convicted of a felony; or a misdemeanor that is specifically listed in "School Safety Legislation."
